  1. To help you achieve the best possible award of SSD benefits, it is necessary that we prove the following:

    Due to the severity, frequency and duration of your impairment's symptoms and limitations, you are incapable of working at any job now and for at least the next twelve months.

  2. In order to begin the claims process, you may complete our free case evaluation questionnaire on our website. The information that you provide to us will be maintained in the strictest of confidence and will only be used to provide you with the percentage of chance that you will receive an award of benefits.

  3. Once we have received your completed questionnaire, we will contact you within 24 hours to provide you with the results of our evaluation. Please keep in mind that when we provide you with the percentage of chance that you will receive an award of benefits, this percentage is arrived at with the understanding that you have utilized our professional disability consulting services.

  4. If we believe that your case can be won, and you wish to retain our disability consulting services, we will forward to you our comprehensive claims package which will include medical authorizations for you to sign permitting the release of your medical records and reports to us, a representation and fee agreement, client information form, as well as other pertinent information about the Social Security disability claims process.

  5. You will then need to complete the client information form listing all of your medical providers and your employers for the past ten years, as well as other important information, review and sign all of the necessary documents in your packet and return them to us for further processing.

  6. Upon receipt of the return of these documents from you, we will forward notice to the Social Security Administration to advise them that we now are representing your interests and we will begin taking immediate steps to process your claim in the quickest most efficient manner possible.
